Helpful information for athletes/parents:
The "History Form" is to be completed by the athlete and/or parents prior to the physical appointment and given/shown to the physician at the appointment.

The "Physical Examination Form" is to be completed by the physician, but will remain with the athlete/parent after it is completed.

The "Preparticipation Evaluation Form" (there are both an English version as well as a Spanish version) is completed and signed by both the physician and the parent, and that is the only form that gets turned in to HCPS. This form signifies that the doctor has completed a physical (which stays in parent possession) and confirms that the student is medically cleared to participate in athletics.

This form MUST be turned in to the coach, the school nurse, or the Athletic Director, they are not to be accepted in the front office per the Athletic Director. It is up to the coaches to relay to all potential athletes when/where/how they will accept forms, which are to be turned in at least a week prior to the start of tryouts.
The "Alternate Transportation Form" is an optional form, to be filled out by parents if/when their child will be utilizing non HCPS transportation to or from an athletic event. Questions regarding this form should be directed to the Coach or Athletic Director in advance of any alternative transportation needs.
